Appointments


Our office does not "overbook" our schedule, therefore your scheduled appointment time has been reserved specifically for you. We do require 48- business hours notice for cancellations.  Cancellations and failed appointments with less than 48 business hours notice are charged a non reversible fee of $95.00.


X-rays


In order for our office to properly manage and diagnosis our patient's oral health care,   X-rays are a requirement.

  • Full Mouth X-rays are taken every 3-5 years. 
  • Check up X-rays are taken yearly.
  • Specific X-rays are taken for services including but not limited to: toothaches, broken, missing or chipped tooth, fillings or crowns.


We are aware that some concerns with taking x-rays include the patient's exposure to radiation.  Our office uses digital x-rays which reduces the exposure significantly. For added patient protection, we also use a lead apron with a thyroid collar.

Financing Options


Payment is due on the day of service.

Payment options:

         Cash, Check, American Express, MasterCard, Visa.
         For major restorative treatment, we do offer financial arrangements.

Finance and Billing fees

A billing fee of $2.00 per month is applied for accounts 30 days overdue. 

Insurance


Dr. Nakamura is a Preferred Provider with Delta Dental Premier.  We also accept most other Dental PPO plans, although we are out-of-network providers  for all plans other than Delta Dental Premier.

Our office is committed to helping you maximize your insurance benefits.  Because insurance policies vary greatly, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts. (Your insurance policy is an agreement between you and the insurance company. If you have any questions regarding your insurance benefits or coverage, you will have to contact your insurance company or refer to your membership benefit booklet.)

Your deductible and estimated portion are to be paid at the time of service. 

As a courtesy to our patients, we will bill insurance companies for services.  If for some reason your insurance company has not rendered payment within 30 days from start of treatment, you will be responsible for the full payment at that time.
